Starvation Before Surgery: How Fasting-From-Midnight Can Slow Your Recovery

What happens to the body when you don’t eat for an extended period of time? 

When you’re nourishing yourself regularly, your body breaks down glycogen, turning it into glucose which it uses for energy. According to Science Alert, about 25% of that energy goes to feeding your brain, while the other 75% powers your muscle tissue and red blood cells. After about six hours of fasting, however, you begin to run out of glycogen stores—and your body has to make a critical shift and begin getting its fuel elsewhere, including from muscle tissue. This eventually leads to muscle breakdown.


Can your body enter starvation mode as a result of fasting before surgery? 

Unless you’re lucky enough to snag the first surgery slot of the day, fasting-from-midnight can often mean you’re stuck waiting for surgery for up to 17 hours with no sustenance—meaning “fasting before surgery” can quickly turn into starvation before surgery. In fact, according to an article published in British Journal of Anaesthesia, a study of 190 patients found that 70% of patients had to fast over twice the recommended length of time for both food and drink—and that 97% were starved longer than required.


While we always recommend you listen to your doctor’s specific instructions regarding fasting before surgery, it’s important to note that fasting-from-midnight is a non-evidence-based practice.   1. But I thought I was supposed to fast before surgery?

First and foremost, you should always follow your doctor’s instructions. That being said, this is definitely the most common question we hear—and understandably so—so let’s break down fasting before surgery versus consuming a pre surgery carbohydrate drink:


  • 150, years ago medical professionals realized having food in your stomach before surgery could cause you to aspirate. 
  • Aspiration is the entry of your stomach contents/food into the lower respiratory tract and lungs—which can be very dangerous during surgery. 
  • In response, medical professionals implemented a new, sweeping rule: patients were to have no food or drink after midnight before their surgery (also called “NPO” or “NPO after midnight” in the medical industry).
  • Fast-forward to the year 2000, when extensive new research (over 30 independent studies) concluded that patients who drank clear fluid before surgery actually had emptier stomachs than their fasting counterparts. 
  • Patients who fasted had a build-up of stomach acids as a result of fasting. Therefore, patients who drank clear fluid prior to surgery were ultimately deemed less likely to aspirate than those who fasted.

So, while you should follow your physician’s specific surgery prep instructions—we do know that modern research points to the fact that consuming a pre surgery carbohydrate drink helps decrease a patient’s risk of aspiration—making the question of a pre surgery drink definitely a worthwhile one to address at your pre-op appointment.   1. When do I need to have my bottles completed by?

    Misconception: “People won’t ‘starve’ if they don’t eat for a few hours before surgery.”

    Reality: Presurgery drinks aren’t just about keeping you from starving.

    Fasting before surgery is a decades-old practice, so it makes sense that patients are wary when they first hear about presurgery drinks. After all, doctors say it’s unsafe to eat or drink before surgery, right? Well, not so much:

     

    Reality: Fasting before surgery is an outdated practice and is being replaced with instructions to nourish and hydrate, instead. 

    We break down everything you need to know about why fasting before surgery is outdated here (so take a look!), but here’s a quick summary: Having food in your stomach before surgery can cause you to aspirate (aspiration is essentially the entry of your stomach contents/food into the lower respiratory tract and lungs, which can be very dangerous during surgery). Because of this, medical professionals implemented a sweeping rule over 150 years ago: no food or drink after midnight before surgery. 

     

    By the year 2000, however, a collection of 30+ independent studies proved that patients who drank clear fluid before surgery actually had emptier stomachs than those who didn’t eat or drink anything—which ultimately reduced their risk of aspiration, making their surgery experience safer than those who fasted. 

     

    When you fast, your stomach creates gastric acids—so it’s not, in fact, empty (even though you haven’t eaten or drank anything). When you drink the right clear liquids up to two hours before surgery, your stomach doesn’t create these acids and is, in fact, empty. 10 Mistakes You’re Making When Preparing for Surgery

When it comes to preparing for surgery, there’s so much to consider. From transportation plans to remembering to pick-up those all-important prescriptions, there are countless different tasks you can easily forget to take care of ahead of time—especially when you’re anxious and overwhelmed about going under anesthesia. To help you on your quest to ensure a smooth procedure and seamless recovery (sans unwelcome surprises!), we’ve rounded up the top 10 mistakes patients make when preparing for surgery. Read on to learn how you can plan for and avoid each. 

 

  1. Putting off buying crucial supplies

Whether it’s a heating pad, ice packs, ace bandages, or medications, properly preparing for surgery means stocking up on everything you’ll need to recover in comfort ahead of time. And, by “ahead of time”, we mean at least 2-3 days before your surgery. Patients often procrastinate when it comes to buying the supplies they need for recovery, only to find the drug store is sold out of them the night prior. You don’t want to be stuck without crucial supplies and meds, so be sure to purchase everything you need a minimum of 48 hours in advance. (This includes filling any post-op prescriptions ahead of time, so you don’t have to worry about waiting for your pain meds to be filled when you need them most upon waking from surgery.)

 

  1. Being too shy to ask your doctor questions

We get it: doctors are intimidating. They tend to use confusing jargon or medical terms we don’t fully understand—and they’re often extremely busy, which can often make us feel like we’re a burden if we ask questions. But a good physician will understand that answering your pre-surgery questions is a crucial part of her or his job. We recommend having a list of questions to ask prepared ahead of your pre-op appointment. Communicate with a back-up driver ahead of time and let her or him know when they can expect to hear from you with the final word on whether or not they’ll be needed.

The “Traditional” Three-Bottle Protocol

Drinking three bottles of ClearFast prior to your surgery is the traditional method recommended by Enhanced Recovery After Surgery Programs (or ERAS® Programs) in hospitals across the U.S. You’ll typically want to take your first bottle of ClearFast 12 hours before your surgery, your second bottle 7 hours before your surgery, and have your your third bottle finished 2 hours before your surgery. So, if your surgery is at 12 p.m., your schedule for drinking clearfast would be:

The “Easy” Two-Bottle Protocol

Drinking two bottles of ClearFast is a common way to prep for surgery as it can sometimes be a bit easier than the three-bottle protocol. (The two-bottle protocol doesn’t require you wake up in the middle of the night to drink ClearFast, as can be the case with the three-bottle protocol, depending on the time of your procedure.) When it comes to how to use ClearFast for our two-bottle protocol, your schedule will be to drink your first bottle 12 hours prior to your procedure, and to have your second bottle completed two hours prior to your procedure. So, if your surgery is at 12 p.m., your schedule for drinking ClearFast would be:

The Iron-Clad Four-Bottle Protocol

For those looking for the absolute best way to ensure they’re offsetting the terrible dehydrating effects of surgery and surgery prep, drinking four bottles of ClearFast is the way to go.
